Customs and Border Protection News for Pets Entering the U.S. as Cargo If a pet owner ships a pet to the U.S. as cargo using a pet shipper, and the pet owner plans on having the pet collected by a professional shipper, the pet owner should download . After completing the appropriate sections of this form, the owner should fax or scan this information along with a copy of the airway bill, the health certificate papers and a copy of the owners driver's license or passport to the pet shipper. The shipper can deliver this form and accompanying papers to the Custom's office at the airport prior to the pet's arrival. In essence, this will mean that the owner is authorizing clearance—not the shipper. This avoids the need to hire a customs broker. A friend or family member may collect and clear the pet without this form because they are not being paid for this service.

NEW CLIENT/PET INFORMATION FORM
Please download, print and complete a Pet Care Information form for each pet that we will be caring for. If you're an existing client and the care instructions have not changed, you do not need to complete this form. If the instructions have changed, please complete this form for each pet. New clients can submit the web form which will provide us with detailed information about you, your pets, and your pet care needs. You can also reach us directly by phone. We will schedule a 'meet and greet' appointment that is convenient for you. New client forms are available for download at the top of this page. We request that these forms be completed prior to our scheduled appointment. During the appointment, we will spend time getting to know you, your pet, discuss your upcoming service, review documents, and finalize all arrangements. Please check our , and call for availability. New customers should print the New Client Packet, and then review & fill in the information requested in ink. Have the completed forms ready at the free Initial Booking Consultation in your home. Please plan to spend 30-90 minutes reviewing your pet care and home care information at the initial consultation appointment.
Next fill out or print an Online Service Request to book service if you have dates planned. Please be sure to include your ADDRESS and PHONE NUMBER in the notes field so that we can locate your home before returning a call.
You and your spouse should read through the agreements listed in the packet and Service Request thoroughly. Only one signature is required on each agreement.
Our pet sitter reserves right to accept changes to reservations only from the clients who have signed the forms, or from any single adult representing themselves as part of the household.
At the consultation you will be given a purple “Shannon’s Pet Sitting” folder to keep you Pet Information Sheet and Veterinary Release Forms in. When we care for your pets you just leave the folder out for us along with any additional notes you would like to leave.
